If you’re here because you’re in need of human milk for your baby, take a deep breath. You’ve come to the right place, because we’re here to help make it safe, easy, and accessible for everyone. All babies deserve the lifesaving nourishment of human milk for the best possible start in life.

For insurance coverage

If you intend to seek coverage or reimbursement for donor milk from your insurance company, we recommend reaching out to your insurance provider initially to confirm whether they cover donated human milk. Upon receiving confirmation, kindly instruct your insurance provider to contact us for a “Letter of Agreement or Authorization.” Please note that your insurance company will likely require a prescription and a letter with supporting documentation from your pediatrician. Kindly ask your pediatrician to contact us for a referral form/prescription, and we will assist you through the necessary steps to initiate the process.

For private pay

If you are privately covering the cost of donor milk, please be advised that no prescription is required. Initiate the process by completing the form below.

Outpatient Request For Milk

Please submit the form below for milk requests with private pay. Recipient Services office hours are between 8:00am – 4:00pm (PST) Monday – Friday.

IMPORTANT: Due to high demand, all orders need to be placed 2-3 weeks in advance of your desired due date and we will do our best to deliver your order as soon as possible.

I acknowledge that Mothers’ Milk Bank is 100% based on donations, and on occasion, the demand may exceed our supply. The milk is not a guarantee. Our first priority are babies in Neonatal Intensive Care Units. A back up plan is essential.
